parish Registration Form
In a parish, the pastor, his assistants, and the faithful work together to build a sense of community, particularly through the celebration of the Mass. To help ensure this dynamic in the parish, the Church requires registration. Parish registration creates a two-fold obligation. On one hand, the pastor is obligated to support the mission and needs of the parish. On the other hand, the individual is obligated to support the mission and needs of the parish. In our society today, sometimes a person may feel more comfortable at a particular parish or like the ambiance of a particular parish even though it is not the territorial parish where the person lives. When a pastor allows these individuals to register, he also accepts the responsibility for their spiritual care. To join Our Lady Queen of Heaven Parish, please fill out the Parish Registration Form below and send it to the parish office.
Direct Donation Form
To make financial stewardship easy, Our Lady Queen of Heaven Parish offers all parishioners the opportunity to set up automatic deposits of your monthly church contributions (known as ACH Direct Debit). Many churches are using direct debit already and parishioners find this to be a convenient, safe, and efficient method of handling their parish support. Simply fill out the Authorization Form below and bring it to the parish office to get started. Even when you fly south for the winter or vacation in summer, you can be sure that the parish will receive your monthly contributions on time. Please contact the parish office with any questions or for more information.
SCRIP Order Form
Buying scrip cards seems confusing but it's really not. We purchase ordinary gift cards, or scrip, from popular retailers at a discount, which the retailers are happy to give because they're guaranteeing themselves customers and some goodwill advertising. Anyone can then buy the gift cards at face value. We keep the difference in price. If you are a parent of an Assumption Catholic Schools student, the profit is used by ACS. When you spend the scrip card at the retailer, the card is worth the full face value; you haven't spent an extra dime to help the parish or ACS, nor have you had to change stores. Scrip is just another way to pay for everyday purchases using gift cards in place of cash, checks, and credit cards. Stop by the scrip table after the weekend Masses or stop by the parish office during normal office hours to purchase scrip.
